呼吸道合胞病毒最早于1955年由美国沃尔特·里德陆军研究所在患有呼吸道疾病的黑猩猩体内发现,1957年Chanock和Finberg从儿童体内分离出,由于其特殊的细胞融合性病变,所以命名为人呼吸道合胞病毒(human respiratory syncytial virus,HRSV)。据报道,HRSV是世界范围内婴幼儿下呼吸道感染最重要的病原体之一,有些地区2岁以内的儿童至少发生过一次感染。近年来,研究发现HRSV也可引起成年人、老年人以及免疫抑制患者下呼吸道感染。2015年全球范围内由HRSV引起的呼吸道感染人数约3.31亿,导致约320万5岁以下儿童住院治疗,其中约6万名住院死亡,而老年人住院约33.6万,住院死亡约为1.4万,医院和社区每年因HRSV死亡人数约为10万~15万。HRSV占全球儿童急性下呼吸道感染的22%,中国HRSV感染率约为31/1000,占儿童急性下呼吸道感染的18.7%。HRSV触发宿主炎症免疫反应,虽然人体对HRSV免疫反应尚不完全,但会引起疾病进一步发展,且HRSV致病机制也尚不完善。HRSV临床尚无有效疫苗,开发有效疫苗和单克隆抗体,即是机遇,也是挑战。随着呼吸道合胞病毒多种基因型相继出现,并呈现交替流行以及共流行的现象,基因型的不断变异、变迁,所造成的疾病程度也有所不同。
