人呼吸道合胞病毒(HRSV)属于肺炎病毒科,正肺病毒属,非节段性单股负链RNA病毒。HRSV主要引起呼吸道感染,已经成为婴幼儿和5岁以下儿童呼吸道感染住院的主要原因之一,也是成年人、老年人以及免疫缺陷患者严重呼吸道疾病的主要病原体之一。 目前,临床尚无有效的HRSV疫苗或治疗方法。 HRSV有A、B两个亚型及多个基因型,不同亚型及基因型流行病学特征不同。 本研究对HRSV病原学、分子流行病学、疫苗研究及HRSV检测分型技术等进行综述,为HRSV疫苗研究及相关呼吸道传染病的防治和监测预警提供重要的理论依据。

HRSV感染后,潜伏期通常为2-8天,临床表现以呼吸道感染症状为主,早期HRSV感染大部分局限于上呼吸道,临床表现如鼻塞、流涕、咳嗽和声音嘶哑等。HRSV感染患儿大多数症状会在1-2周内自行消失,少部分可以发展为下呼吸道感染,主要表现为毛细支气管炎或肺炎,多见于2岁以下婴幼儿,临床症状包括咳嗽、喘息,极少部分进一步加重出现呼吸急促和喂养困难等,严重者可发展为呼吸衰竭。
各年龄段人群均可感染HRSV,但HRSV感染的高危人群主要集中在儿童、老年人和免疫功能低下人群。
在传播途径方面,有症状的HRSV感染者和无症状的隐性感染者都可传播病毒,通常可通过以下途径传播:一是接触传播,如鼻咽黏膜或眼黏膜接触到含病毒的分泌物或污染物;二是近距离接触患者咳嗽或打喷嚏等。北京大学人民医院呼吸内科主任高占成表示,对于易感人群的预防,要注意勤洗手,避免不洁净的手触摸眼、口、鼻,养成良好的个人卫生习惯,及时佩戴口罩,减少出入人员密集场所。
