不管是前交叉韧带还是半月板术后做一样的康复就行了！！！ 这是错误的！！！
无论是在康复师的角度也好，还是在患者的角度也好，我们会发现在大部分人在进行半月板或者前交叉韧带术后的康复都是相近的！几乎没有什么区别！然而这是正确的吗？
这是错误的！！！
在前交叉韧带损伤与半月板损伤里对于患者的术后康复方案虽然在一定程度上有很多重合的地方，但是在对于手术部位的保护以及对于运动的选择上有着不一样的治疗观点，阅读本文的意义对于康复师来说在选择康复方案时能尽可能的规避风险，对于患者来说减少无用的治疗方案，减少治疗的时间与金钱，从而达到最佳的治疗效果。
在物理治疗的范畴里面对于术后康复我们需要注意一些什么

保守治疗与手术治疗的选择
半月板：
在半月板中临床的一线治疗是保守治疗（非甾体抗炎药，物理治疗）为主
根据文献资料显示：在退行性半月板撕裂中，物理治疗在 5 年的膝关节功能方面不亚于关节镜下部分半月板切除术。物理治疗有着不亚于手术治疗的治疗结果。

前交叉韧带：
在前交叉半月板中大多数人都会推荐进行手术治疗

文献资料显示：大多数术后膝关节稳定性研究发现，前交叉韧带重建术后的膝关节功能优于保守治疗。保守治疗中因为前交叉韧带损伤导致约束胫骨前移的力减少，作为二级约束的半月板获得了更多的压力，加速了半月板以及软骨的磨损，增加了关节炎的几率

前交叉韧带重建术后（ACLR）术后早期（1~4周）
对于运动的选择
半月板：
在半月板损伤中前期的闭链训练是相应的较少的，闭链运动会对于修复的半月板产生一定的压力。
前交叉韧带：
前交叉韧带主要控制而对于前交叉韧带重建来说，开链运动对于前交叉移植物的压力是很大的
开放动力链 (OKC) 练习存在争议，因为它们可能会对移植物造成压力。OKC 膝关节伸展练习的初始限制可能从 ROM 开始到 90° 到 45°，以防止移植物损伤，随着移植物成熟而进展到完全 ROM。

前交叉韧带的生长期（增殖期）在术后7-21天，韧带在第5-21天时是最脆弱的
支具的使用
半月板修复支具的使用：
患者进行了半月板修复，则建议穿戴膝关节支具或膝关节固定器以防止膝关节在负重时弯曲，这可能导致修复的半月板再撕裂。膝关节固定器可使膝关节完全伸展，而铰链式护膝可以控制膝关节的运动。
前交叉韧带支具的使用：
前交叉韧带重建后伸直固定很有必要，这样对于新固定的韧带不会造成压力，使关节有时间正常愈合。
对于支具的话，可凭个人意愿购买。研究表明支具没有多大的治疗意义，佩戴膝关节支具可以减少 ACL 损伤患者的不稳定感，但最初使用支具也会导致日常生活活动中出现更多不适。
负重时间
前交叉韧带：
前交叉韧带术后康复指南上写到，ACL重建术后可在耐受的情况下即刻负重。有数据显示即可负重有利于髌股关节疼痛的减少， 35% 降至 8%。
半月板：
半月板修复术后康复指南里提到：修复术后的患者在2~6周往往需使用拐杖进行无负重或者部分附中
在损伤部位的撕裂类型上也有不同的负重要求：
外周修复往往只需要2周的时间就可以部分负重在3~4周内就可以在可接受的不适下负重行走。
而复杂性的撕裂则需要更久的时间6周部分负重，7~8周可耐受负重
加速康复方案：
在最新的研究中指出加速康复方案，在可以耐受的情况下尽早完全支撑，无需支具并立即活动手术膝关节。Shelbourne和 Mariani报告说，在接受加速康复方案和常规康复计划的患者中，半月板愈合的百分比很高，但加速组在全范围运动中表现出更快的恢复（加速组 6 周与标准组相比 10 周），2 个月时股四头肌强度更高（加速组 82% 与标准组 71%），并且加速恢复完全活动（加速组 10 周与标准组 20 周）标准组）
总结
在两者的术后物理治疗中都强调尽早地进行活动
在对于前交叉韧带重建术后与半月板缝合康复计划的统计学研究中显示早期运动是安全的，有助于避免后期关节纤维化的问题。对选定的有动机的患者进行最低限度监督的物理治疗是安全的，没有明显的并发症风险。
在 4 个月内每周进行 3 次体育锻炼可使膝关节功能改善 35% 以上。
我们拒绝标准化“食谱”式的治疗，每个患者情况都是不同的，对于康复的反应也是不同的，每位患者都是独立的个体，任何康复计划都应该以患者目标为中心。物理治疗师是运动专家，我们会根据您的身体情况个性化定制诊疗方案。
